Insurance Information

We accept most medical insurances: Aetna, Blue Cross Blue Shield, Cigna, Health Partners, Humana, Medica, Medicare, United Health Care and UMR. We accept VSP and EyeMed vision insurances. We accept HSA/FSA dollars. We are happy to help if you have insurance questions.

Eyeglasses and Contacts

At any visit, please bring all glasses you are currently using including reading glasses, computer glasses and sunglasses. Contact lens packaging is also a helpful start. Also, remember to bring any eye drops that you are currently using.

Health History

To improve efficiency, after scheduling an email will be sent to you with a secure link to a health questionnaire. This eliminates most of the paperwork needed at your appointment.

Dilation

The use of eye drops to dilate your eyes may be necessary to get a complete health examination. Infinity Eye Care is offering Optomap imaging, which can be an alternative to dilation for many patients.
